Add and deploy a survey using a collection link

This article will show you how to add a simple feedback questionnaire and collect responses with a collection link.

Before you start

You must have permission to add databases to follow this tutorial. If you don’t have this permission, you can sign up for a free trial of ActivityInfo.

Add a database

After logging into database, click “Add database”
Enter “My survey database” as the database name
Select “blank database” from the list of templates

Add a form

Enter “Feedback survey” as the Form name

Add the name field

Click “Add a field”
From the list of field types, click “Text”
Enter “Your name” as the Label
Enter “Providing your name is optional” in the Description field
Click “Done” to close the field card

Add the satisfaction field

Move your mouse cursor to below the first question, and a “+” button will appear
Click the “+” button to add another field
From the list of field types, click “Single selection”
Enter “How satisfied were you with the service you received?” in the label
Check the “Required” checkbox in the list of Settings
Enter “Satisfied” in the first row in the Options section
Click “Add option” to add another Option
Enter “Neither satisfied nor dissatisifed” in the second row
Click “Add option” to add another Option
Enter “Dissatisfied” in the third row
Click “Done” to close the card for this field.
